New Patient Registration Form
To help us prepare for your first appointment, please download and complete the New Patient Registration Form in advance. You can submit it with your enquiry below, or bring it along to your first appointment.
GP Referral Required: To schedule an appointment and be eligible for a Medicare rebate, a written referral letter from your GP is required. Please attach the referral letter — including the reason for the consultation — to help our team assess whether we can meet your expectations and determine if we are able to offer an appointment.
Emergency: We do not provide emergency psychiatric services. If you or someone close to you is experiencing distress or is in immediate danger, please call 000 or go to your nearest hospital emergency department.
